Precinct Election Official Survey

Please complete this survey and submit it to us within two weeks after the election in order for your information to be included in our post-election report. If possible, please include your name, as it assists us in the documentation of events related to each voting location. However, you must choose your work location from the drop-down box in order for the survey to be submitted. Mark the circle that best reflects your feeling about each statement and add any additional comments. (If you have already submitted a paper copy received on Election Day, you do not need to complete this survey again.)
